If you suspect that your card has been lost or stolen, you must let us know.
For lost cards: Submit a Statutory Declaration
For stolen cards: Submit a statutory declaration, and A police report

Step 1 - enter your MSIC number and press Step 1.
Step 2 - is to print out your Lost Card information Pack.
