Reporting a Lost or Stolen MSIC

If you suspect that your card has been lost or stolen, you must let us know.

  • For lost cards: Submit a Statutory Declaration
  • For stolen cards: Submit a statutory declaration, and A police report

  • You can also contact ClientView Pty. Ltd. directly on 1300 855 960 if you wish.

  • Step 1 - enter your MSIC number and press Step 1.
    Step 2 - is to print out your Lost Card information Pack.
